We are a participating provider with many insurance plans. If you would like to change your health provider to Equity Health, just give our staff a call at (415) 503-6000 or contact your plan directly to verify that you are assigned to our clinic as a medical home.
Do not have insurance?
Our Enrollment Department can provide assistance with health insurance enrollment.

Need payment assistance?
Patients who do not qualify for Healthy San Francisco, Medi-Cal, Medicare, or Family PACT may qualify for the sliding fee discount program. Equity Health has a sliding fee discount program to provide essential healthcare services for low-income patients who cannot afford care, lack sufficient insurance, or do not qualify for any other program. It allows patients to pay a reduced fee to cover basic comprehensive health services. Discounts are offered based on family size and income. Please bring your proof of income with you to the visit to speak with an Eligibility Specialist.

How to Prepare for Your Visit
For your visit, we ask that you bring:
- Bring your Photo ID
- Health Insurance Card
- A list of all your symptoms, health concerns, and allergies
- Arrive Early
Don’t be afraid to ask questions. It is important to us that all your concerns, no matter how small they seem to you, be addressed–be your own advocate.
Please arrive 15 minutes prior to scheduled appointment.
